Scheduling an Appointment and Patient Registration

Before scheduling your appointment, we would like to help you organize your information and complete all of the required forms to ensure a quick and easy registration process. The majority of registering process can be accomplished at home and on line for your convenience.

Providing Patient and Insurance Information

Before calling or scheduling an appointment on line, have tour insurance information ready which includes:

Name of the Insurance Company and insurance plan
Names of Supplemental Insurance and plans
Policy and Group numbers
Name of your Primary Care Physician and Prescription

Your Doctor's Prescription

Make sure to have your doctor's prescription available for the following information:

Name of the referring physician
His or her telephone number
Other information possibly needed

Completing Patient Forms

After making your first appointment, you will be asked to complete new patient forms and Intake Surveys. All of these forms can be downloaded and completed at your convenience. Make sure to bring the completed forms with you to your appointment allowing more time for you to be with the therapist. Click here for the Patient Forms.
